  • Part A:

  • Part B:
    • Reiterates details from Part A and includes information on members eligible for children/widow pension.

Form 5: Monthly Report for Newly Enrolled Employees

Form 5 is a monthly report detailing employees newly enrolled in the provident fund scheme. Information includes organization details, employee account numbers, names, dates of birth, and track records. The employer must file and stamp the form with the date of filing.

Form 10: Monthly Report for Ceased Employees

  • Form 3A:
    • Month-wise contribution details to the EPF and Employee Pension Fund.

  • Form 6A:
